The MSHO Managed Care Coordinator, RN provides case management and coordination of care for designated health plan/MCO members with complex health and social needs! This person will conduct comprehensive health assessments of each member's bio-psycho-social and environmental safety needs. Lead utilization of services to achieve efficient outcomes with the most efficient manner to meet the member's needs! Ensures all CMS & State regulatory requirements are met in a timely manner.

MSHO Managed Care Coordinator, RN- Overview

Key responsibilities include but are not limited to:

  • Carries a client caseload, completes Health Risk Assessments in the community, develops individualized Support Plans and assists in preventing unnecessary hospital and nursing home admissions allowing the member to remain in their community with all the vital supports available
  • Incorporate an interdisciplinary/multidisciplinary and preventive focus to address concerns and resolve actual or potential health concerns
  • Evaluates, documents and monitors member's health and wellbeing through annual and interim assessments, phone calls, throughout patients care
  • Facilitates timely referrals to Managed Care Specialists to identify service providers, initiate services and process authorizations
  • Knowledgeable in taking care of emergent situations as well as chronic disease processes and member needs throughout the continuum of care
  • Identifies areas of growth and needs and shares with management to implement improvements within policies and procedures
  • Assists in the education and development of family medicine clinics and residents
Minimum Qualifications:
  • Licensed RN in the state of Minnesota
  • Minimum of 1 year home and community based service experience
  • Minimum of 2 years experience in MSHO/MSC+ Care Coordination
  • Strong digital literacy in Microsoft Word, Excel, Access, Outlook, and Adobe Professional
  • Reliable Transportation for home visits
  • Home Internet hook-up for laptop

Location: Remote with some on-site patient expectations

Hours: 1.0 FTE
