POSITION SUMMARY

Responsible for planning, managing, implementing, and evaluating the operations of the Department of Case Management which includes oversight of effective resources utilization and the provision of high quality and excellent care,

Work in collaboration with administration and physician leadership to establish measurable standards and outcomes,

Oversee nurse case management and social work services,

Ensure continued development of the program to improve organization performance to enhance provision of services

MINIMUM QUALIFICATIONS AND REQUIREMENTS

Three years of clinical experience required,

BSN preferred,

CCM or ACM certification preferred

ESSENTIAL DUTIES AND RESPONSIBILITIES

Supervises case management operations of the inpatient medical and surgical units,

Provides support and education to case management staff, including oversight of CEU programs and training,

Offers clinical supervision as needed,

Identifies need for interdisciplinary or family meetings and facilitates these meetings,

Collaborates with department leadership to streamline processes and implement strategies to create more efficient delivery of care,

Ensures compliance with regulatory documentation standards,

Understands financial responsibilities of the case management department and safeguards system resources by actively participating in utilization management and insurance communication,

Collaborates closely with physician advisor,

Participates in interdisciplinary rounds and facilitate communication with the interdisciplinary care team,

Completes psychosocial assessments and implement patient-centered care plans that meet needs identified in assessments,

Discharge planning activities: seamless and timely coordination of care including: referrals to home care, DME, SNF/ rehab placements, hospice, community resources, and transportation arrangements,

Participates in quality data collection and risk management referrals

Collaborates with department leadership regarding ongoing performance management of front-line social workers,

Ensures that staff operate within compliance with all applicable policies, laws and regulations,

Participates in quality data collection

OTHER DUTIES AND RESPONSIBILITIES

Any and all other duties as assigned by Manager, Regional Director, or Senior Leadership Team,
