Qualifications

Education: Doctor of Pharmacy or Bachelor of Science in Pharmacy degree
Experience: Completion of one year pharmacy residency.
Equivalency: One year of pharmacy work experience accepted in lieu of residency.
Licensure: State of Colorado Department of Regulatory Agencies (DORA) Pharmacist license

POPULATION SPECIFIC CARE

ESSENTIAL FUNCTIONS
An employee in this position may be called upon to do any or all of the following essential functions. These examples do not include all of the functions which the employee may be expected to perform.
* Provides drug information services by responding to questions regarding medication compatibilities, adverse drug events, regimens, dosing, interactions, storage and disposal, compounding, therapeutic uses of medications and pharmaceutical supplies in a timely manner; using a systematic approach to effectively searching, retrieving, and evaluating the literature and appropriately communicating and applying the information to the patient care situation; and assisting with the development, maintenance, and implementation of hospital and department policy and procedures related to medication use, storage, restrictions, and administration.
* Sustains and implements quality and safety initiatives and practices in the department and throughout the hospital. Reports adverse drug events and medication variances using the hospital variance reporting program (QSRS).
* Sustains clinical pharmacy services throughout the organization by providing pharmaceutical care for pediatric patients of all age ranges through participation in multi-disciplinary team rounds and/or care, emergency response, education, guidance, interpretation and analysis of medication related therapeutic issues; documenting pharmaceutical interventions according to departmental standards; performing therapeutic drug monitoring and disease outcome analysis; and timely verification of medication orders in an electronic health record system.
* Participate in general pharmacy operations throughout the organization through appropriate medication preparation, dispensing, and inventory control. Provides oversight to pharmacy technicians, interns, residents and students as needed.

Guidelines: Fully qualified, full performance or journey level. Guidelines are generally but not always clearly applicable, requiring the employee to exercise judgment in selecting the most pertinent guideline, interpret precedents, adapt standard practices to differing situations, and recommend alternative actions in situations without precedent.
Complexity: Duties assigned are generally complex and may be of substantial intricacy. Work assignment is performed within an established framework under general instructions but requires simultaneous coordination of assigned functions or projects in various stages of completion.
Decision Making: Exercises judgment and discretion, and is responsible for determining the time, place and sequence of the work performed
Communication: Contacts with team members, clients or the public where explanatory or interpretive information is exchanged, defended, and gathered and discretion and judgment are required within the parameters of the job function.
Supervision Received: Performs work with minimal supervisory oversight. Under general supervision, the employee receives assignments and is expected to carry them through to completion with substantial independence. Work is reviewed for adherence to instructions, accuracy, completeness, and conformance to standard practice or precedent. Recurring work clearly covered by guidelines may or may not be reviewed.

Salary Information

Annual Salary Range (Based on 40 hours worked per week): $121,394.93 to $182,092.39
Hourly Salary Range: $58.36 to $87.54
