Responsibilities

A-Team Solutions LLC is seeking Pharmacist for a full-time contract position at the Phoenix VA Medical Center. Outpatient pharmacy services are needed ASAP and shifts are: 9:30a -6p or 8:30a-5p. Weekend and holiday coverage may be needed pending staffing needs.

Duties and Responsibilities:

  • Display a cooperative attitude and effectively work in a team dynamic to implement the mission and goals of the VA organization mission and goals.
  • Have the physical ability to lift, carry, and stock pharmacy supplies. This includes transport of supplies to wards, cache, or individual prescriptions to the dispensing window. Prescriptions may range from individual bags to cases of product or supplies up to 50 lbs. Must be able to stand, walk, climb stairs, and procure product from high or low shelving areas. Must be able to perform these duties while standing for their entire shift and maintain safe body mechanics especially while moving heavy items.
  • Have experience in outpatient, inpatient functions, and clinical functions (including medication reconciliation and residency preceptor abilities). This would include but not be to: limited proficiency in drug distribution and control, pharmaceutical care services, pharmacy practice management. Areas of emphasis include:
  • Drug Distribution
  • Prescription Verification and distribution.
  • Unit dose medication preparation, verification, and dispense.
  • Controlled substance dispensing.
  • USP 797 compliance for sterile compounding and IV room proficiency (IV admixture, TPN, Chemo,
  • Discharge Counseling and Patient Counseling. Adapting counseling information to the understanding of the customer.
  • Pharmaceutical Care Services
  • Monitoring of drug therapy
  • Providing Drug Information.
  • Medication reconciliation skills
  • Maintaining Formulary Criteria and general Formulary
  • Contracted staff will complete all required elements of the CPRS note entry process including location, encounter data, and adverse event reporting.
  • Functions as a mid-level provider to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas.
  • Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Follows up with patients on lab or test results to discuss the plan of therapy including changes in medication therapy, referral to primary care or specialty provider.
  • Adjusts medication regimens based upon pharmacokinetic and pharmacodynamics data.
  • Follows-Up with patients on lab or test results to discuss the plan of therapy, i.e. changes in medication therapy, monitoring, and additional testing requirements.
  • Monitor and report drug errors, adverse drug reactions, allergies, and patient compliance issues. Document findings per facility procedures. Actively participates in the VAADERS Adverse Drug Reaction (ADR) reporting system.
  • Performs medication reconciliation and informs healthcare providers of any discrepancies noted
  • Reviews and evaluates requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable and as assigned.
  • May serve as preceptor for pharmacy students and pharmacy residents in accordance with established protocols.

Requirements

Requirements:

  • Be a citizen of the United States.
  • Pharmacy graduate from an American Council on Pharmaceutical Education (ACPE) approved college or university pharmacy degree program.
  • Have a current and unrestricted Pharmacist and Pharmacist Controlled Substance license in good standing in a state, territory, commonwealth of the United States or the District of Columbia.
  • Have a National Provider Identification (NPI) number
  • Have immunization certification
  • Have a NABP e-profile number.
  • Be proficient in speaking, writing, and understanding the English language.
  • Be proficient in communicating with patients and staff in a professional manner. This includes customer service skills, phone skills, and medical record documentation skills.
  • Be proficient in typing, keyboarding, and using office equipment
  • BLS certification through American Heart Association
