As a Care Coordinator - RN you will provide a deliberate patient and family centered partnership with the healthcare team by developing and supporting a coordinated plan of care between visits (Inpatient and Outpatient) to promote a smooth transition for patients and families across the healthcare continuum.
For 70 years, our Cardiovascular Thoracic Surgery program has led the way in heart care, transforming lives with major advances in open heart surgery, valve replacements, transplants, cardiac resuscitation, and other techniques to treat heart disease. Our team of surgeons, advanced practitioners, nurses, and healthcare professionals bring together a deep understanding of academic medicine and advanced clinical care to tailor our therapies to each patient's individual condition. The Cardiovascular Thoracic Surgery Team is looking to add a team member who values team collaboration and has a passion for helping others.
The Care Coordinator - RN works to maximize the patient's health outcomes and experience by identifying patient and family needs, completes pre-visit planning, supports clinic visits, and provides closure to the clinic visits. The Care Coordinator - RN utilizes coaching to help ensure patient and family participation in treatment plans and provides exceptional customer service to internal and externalcustomers (providers, clinic staff, patients, and families).
