SCHA is a county-based health plan serving 12 counties in Minnesota. These counties are Brown, Dodge, Freeborn, Goodhue, Kanabec, Morrison, Sibley, Steele, Todd, Wabasha, Wadena, and Waseca.
Programs:
AbilityCare (HMO SNP) is a Special Needs Basic Care program designed for people with disabilities who live in our 12-county service area and are eligible for Medical Assistance only or are eligible for Medical Assistance and Medicare Parts A and B.
SeniorCare Complete (HMO SNP) is a Minnesota Senior Health Options program for those age 65 or older that live in our 12-county service area, and are eligible for Medical Assistance and Medicare Parts A and B.
Minnesota Senior Care Plus (MSC+) is a program for those age 65 or older that live in our 12-county service area and are eligible for Medical Assistance.
Prepaid Medical Assistance Program (PMAP) is a health care program for families and children.
MinnesotaCare is for those age birth and up, that live in our 12-county service area. There is a premium for each month of enrollment that is based on income, and determined by the state.
